Summary
of duties and responsibilities:
The SLS Staff Personal Attendant and/or AST Personal Attendant reports
directly to the SLS and/or AST Supervisor or Director and is responsible for
the daily care of adults with developmental disabilities. Duties include but
not limited to the following:

Feeding
·
Dressing
·
Bathing
·
Assisting with Hygiene
·
Scheduling of Medical Appointments
·
Assisting with Meal Preparation
·
Money Management
·
Shopping for groceries or other personal Items
·
Assisting with the use of a telephone
·
Supervise clients while on various outings
·
Ability to lift, push or carry with
assistance up to 75 pounds or more.
·
Transferring/lifting of the client to and
from chair, bed, car, toilet, bathing, etc. without limitations
·
Perform housework when such activities are
related to the supported living of the person and cannot be performed by him or
herself alone due to a health, disability or age limitation.
·

·
Participate in the development of the
individual program plan (IPP) and/or individual service plan (ISP) and/or
individual educational plan (IEP).
·
Provide training as outlined in the IPP,
and/or ISP, and/or IEP.
·
Communicate on a regular basis with
InClusion Administration, client and/or families and other services providers
and agencies, as needed.
·
Planning, coordinating, and evaluating
assigned staff efforts to achieve the programs objectives and client outcomes.
·
Assessing the programs effectives in
achieving the client IPP objectives and identifying barriers to client success
and how those barriers can be overcome.
·
Maintain a clean, safe and healthy environment
·
Report special incidents according to
InClusion Services policies and procedures.
·
Assist, guide, train and support in
“skill” development relating to client’s needs.
·
Ensure
that the client
receives professional, quality
services in a safe and meaningful manner.
·
Teach clients how to increase
their activities of daily living at home or in the community.

Monitors
services and clients
progress in relationship to established goals
and objectives per client’s plan
·
Attends team meetings or other training as
part of continuation of education or any agency –wide training/in-services programs
·
Participates or provides information in
client’s quarterly/semi-annual/annual meetings.
·
MUST have phone ON at all times during
work hours and MUST check in and out during work hours via text, email or phone call.
·
MUST complete time sheets accurately and
legibly, and be signed-off by parent/consumer at all times and submit paperwork
on Pay Day (see Pay Day Schedule).
·
MUST complete weekly
notes or other notes accurately and legibly, and submit paperwork on Pay Day.
·
Shall complete the following hours of
annual training: 12 hours of annual training (per program design and annual evaluation) every year.
·
Must submit requirements and have current
requirements active and up to date (First Aid, CPR, CPI, Car Insurance and
Registration, and Driver License, etc)
·
Complete other job related duties and
assignments as required to meet the goals and objectives of the assigned department/s.
