Provide personal care and related services in the home or in a skilled nursing facility
Promote client's mental alertness through involvement with activities
Report any changes in client's mental or physical condition
Perform routine housekeeping tasks as related to a safe and comfortable environment for the client
Collaborate with IDT and families to promote optimal care
Assist the client to maintain good personal hygiene and maintaining a healthy safe environment
Hearing, eyesight and physical dexterity must be sufficient to note changes in client condition and coherently report changes to supervisor
Must be able to communicate clearly in both verbal and in writing
Must understand and respect clients - including ethics and confidentiality of care
