Urgent Home Care RN (FT)
RN Full Time 40 Hrs / Week Home Care
DUTIES / RESPONSIBILITIES:
- Clinical Practice: Evaluates, implements and develops a home care treatment plan for participants.
- Monitors participant changes and adjusts treatment plan accordingly.
- Provides skilled nursing care as needed.
- Supervision and Training: Instructs and supervises geriatric aides and LVNs in carrying out the home care treatment plan.
- Teaches and counsels participants and family members regarding the participant’s care and safety.
- Interdisciplinary Teamwork and Communication: Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem-solving.
- Communicates effectively with participants, family and team members.
- Other related duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities):
- Graduate of an accredited college or university with either an associate or baccalaureate degree in nursing.
- Possess a current California Registered Nurse license. A California Public Health Nurse certificate is desired.
- Experience in public health nursing desired.
- One-year of experience working with the elderly or frail population is required.
- Ability to work as part of a multidisciplinary team.
- Two years of acute care experience required (ICU, ER, or Critical care) or homecare experience
- CA Driver’s license and be WILLING to drive and travel.
- CPR certificate

Duties and Responsibilities:
- Work with various clients, such as the elderly, children, the physically disabled, the cognitively impaired, and the chronically ill.
- Cook, dress, bathe, and assist clients in using the restroom.
- Administer first-aid or CPR to elderly/children when needed.
- Maintains a clean, safe, nurturing, and stimulating environment
- May include additional household tasks, such as cleaning, cooking, housekeeping, laundry, and pet care.
- Remind clients to take medication.
- Transport clients to appointments or outings.
- Up to 40 hours per week.
Requirements and Qualifications:
- High school diploma or equivalent preferred (Caregiver)
- Must be a Certified Nursing Assistant (CNA)
- At least 1-year experience working with the frail population, elderly, and children
- Attend training and pass a standardized exam.
- Knowledgeable in basic emergency and safety techniques. CPR, BLS, and First Aid
- With a valid Driver's license
- Be in good physical condition.
- Reliable transportation.
- Flexible schedule.
